What is an Advanced Care Directive?

An Advanced Care Directive contains specific instructions for a person's future healthcare, including the use of life-sustaining treatment.

As a legal document, an Advanced Care Directive will only operate if a patient is no longer able to make decisions for themselves.

As part of the directive, patients are able to appoint a decision-maker who will advocate for their wishes and make decisions on their behalf.

The document can include instructions about end-of-life care, living arrangements, personal issues and resuscitation.

Bringing or setting up an Advanced Care Directive

If you already have an Advanced Care Directive in place, please bring this with you to hospital and give us this information when you arrive.

If you would like to discuss Advanced Care Directives on admission, or during your consultation in the clinic please speak to your healthcare team.
